Botox, Filler, and Sculptra After 60: What Actually Works
After 60, what works best is usually a combination: a structural hyaluronic acid filler to replace lost support in the cheeks and jaw, Sculptra to rebuild collagen gradually, and a light, careful hand with Botox. There’s no single best filler for older women, because the right product depends on where you’ve lost volume and how much your skin has thinned. The goal is to look rested and like yourself, not filled.
We see a lot of patients in their 60s and 70s, including plenty of snowbirds who book their maintenance around the months they’re in Orlando. Many tell us the same thing: they’ve avoided injectables because they’re afraid of looking puffy or “done.” That fear is fair. Here’s how we approach the mature face so that doesn’t happen.
What Changes in the Face After 60
Wrinkles are the part everyone notices, but they’re not the whole story. By this stage, the bones of the cheeks, jaw, and eye sockets have slowly lost projection, so the skin has less to drape over.
At the same time, the fat pads shrink and shift. The cheeks flatten and some fullness slides lower, which deepens the folds around the mouth and creates jowls.
Then there’s the skin itself. Collagen and elasticity drop, and years of Florida sun don’t help. Skin can look crepey and doesn’t bounce back the way it used to.
This matters because chasing each line with filler doesn’t work well at this age. Filling a deep fold without restoring support above it is how people end up looking heavy. We’d rather rebuild the scaffolding first and let the lines soften as a result.
The Best Filler for Older Women Depends on Where You’ve Lost Volume
Different hyaluronic acid fillers have different textures. Firmer, more lifting gels go deep along the bone; softer, more flexible ones go where the skin is thin and moves a lot. Here’s roughly how we match them up.

| Area | What it does | Products we often consider |
|---|---|---|
| Cheeks and midface | Restores lift and support | Juvéderm Voluma, Restylane Lyft, Restylane Contour |
| Jawline and chin | Sharpens the jaw edge, reduces the look of jowls | Juvéderm Volux; Restylane Defyne for the chin |
| Smile lines and marionette lines | Softens folds once support is back | Restylane Refyne or Defyne, Juvéderm Vollure |
| Lips and lip lines | Subtle hydration and edge definition | Juvéderm Volbella, Restylane Kysse or Silk |
| Back of the hands | Replaces volume over veins and tendons | Restylane Lyft |
That’s a starting map, not a menu. Your nurse practitioner will choose based on your skin thickness, your anatomy, and how much movement the area has. You can see the full range on our dermal fillers in Orlando page.
A note on under-eyes: hollows are common after 60, but thin skin and puffiness make this a tricky area. Sometimes filler helps; sometimes it makes bags look worse, and we’ll tell you if you’re in the second group. If lips are on your list, our Day 24 post on lip filler over 40 goes deeper on keeping results natural.
Where Sculptra Fits
Sculptra is a different kind of injectable. Instead of adding volume right away, it stimulates your own collagen over several months. That makes it a great match for the mature face, where the overall quality and thickness of the skin has dropped, not just one spot.
The tradeoff is patience. Sculptra is usually done as a series of sessions spaced weeks apart, and results build gradually over a few months. It also can’t be dissolved the way hyaluronic acid filler can, so we’re deliberate with placement. Botox After 60: Less Is Usually More
Botox still works well later in life, but the dosing and placement change. Many people over 60 rely on their forehead muscle to hold up heavier brows or hooded lids. Freeze that muscle completely and the brows can drop, which makes the eyes look more tired, not less.
So we tend to go lighter in the forehead, focus more on the frown lines and crow’s feet, and look at areas like the neck bands and mouth corners. If you’re newer to Botox, our post on starting Botox at 40 or 50 covers a lot of the same thinking. Static lines that are etched in at rest won’t fully disappear with Botox, which is where filler and skin treatments come in.
When Injectables Aren’t Enough
Here’s the candid part. Fillers and Botox can do a lot, but they can’t tighten significant loose skin on the neck or lift heavy jowls the way surgery can. If you’d need a lot of filler to get the result you want, that’s often a sign a surgical consult is the smarter move. We cover the differences in filler versus facelift.
Skin quality also matters as much as volume. Microneedling, chemical peels, and laser treatments can improve texture, sun damage, and fine lines in ways injectables can’t. Many of our older patients get the most natural results by pairing a little structure with better skin.
One practical point: if you take blood thinners or aspirin, tell us. You’ll likely bruise more, but please don’t stop any prescribed medication without your prescriber’s okay.
Quick answers
What is the best filler for older women?
It depends on the area. Firmer fillers like Voluma or Restylane Lyft work well for cheek support, while softer ones like Volbella suit lips and fine lines. Sculptra is often added to improve overall skin thickness.
Is it too late to start fillers at 60 or 70?
No. Plenty of people start later in life and do beautifully. The key is a conservative plan that restores support rather than chasing every line.
Can fillers make older women look worse?
They can if too much is used or it’s placed in the wrong spots, which is where the puffy, overfilled look comes from. Thoughtful placement and gradual sessions prevent that, and hyaluronic acid filler can be dissolved if needed.
Is Sculptra or filler better for mature skin?
They do different jobs. Filler gives immediate structure in specific areas, while Sculptra improves collagen over months. Many older patients do best with both.
